Magnesium: The Deficiency That's Silently Running Your Body Down
The Scope of the Problem
Magnesium is an essential cofactor for over 300 enzymatic reactions — including every reaction that creates or uses ATP. Yet population surveys consistently show that 50–60% of adults in Western countries fail to meet the recommended dietary intake, and RDA itself is considered by many functional medicine clinicians to be too low to achieve optimal cellular function.
The gap between 'not deficient' and 'optimal' is clinically significant. Standard serum magnesium testing captures less than 1% of the body's total magnesium — the vast majority is intracellular. A patient with suboptimal intracellular Mg²⁺ can have a 'normal' serum result while experiencing the full spectrum of deficiency symptoms.
Sleep, HPA Axis, and Stress
Magnesium regulates the HPA (hypothalamic-pituitary-adrenal) axis and modulates NMDA glutamate receptors in the brain — the same receptors implicated in anxiety, hyperarousal, and insomnia. In a double-blind RCT by Abbasi et al. (2012, Journal of Research in Medical Sciences), supplementation with 500mg/day of magnesium oxide in elderly subjects with insomnia significantly improved:
- Sleep time and sleep efficiency
- Serum melatonin levels
- Serum cortisol (reduced)
- Serum renin and aldosterone (normalized)
For stress resilience, a meta-analysis of 18 RCTs by Boyle et al. (2017, Nutrients) concluded that magnesium supplementation significantly reduced subjective anxiety measures, with the strongest effects in populations with pre-existing hypomagnesemia.
Formulation: Why Glycinate and Malate Outperform Oxide
Not all magnesium forms are equal. Magnesium oxide — the most common commercial form — has poor bioavailability (approximately 4%). Magnesium glycinate (bound to glycine) and magnesium malate are significantly better absorbed and have the added benefit of substrate provision: glycine supports sleep (via glycine receptor in the CNS), and malate supports the Krebs cycle, making magnesium malate particularly useful in fatigue protocols.
AXEL Lipo® MgB6 combines magnesium in optimally bioavailable form with B6 (as P5P, the active cofactor form) — recognizing that B6 is required for the enzymatic conversion of dietary magnesium to its intracellular active form.
