The 12 Hallmarks of Aging: A Framework for Longevity Interventions
The Original Framework
In 2013, Carlos López-Otín and colleagues published 'The Hallmarks of Aging' in Cell — a paper that has since become the most cited work in aging biology. It identified nine interconnected molecular processes that drive biological aging: genomic instability, telomere attrition, epigenetic alterations, loss of proteostasis, deregulated nutrient sensing, mitochondrial dysfunction, cellular senescence, stem cell exhaustion, and altered intercellular communication.
In 2023, the framework was updated in a revised Cell paper to include three additional hallmarks: disabled macroautophagy, chronic inflammation ('inflammaging'), and dysbiosis — reflecting a decade of new evidence, particularly from microbiome science.
The 12 Hallmarks and Their Clinical Correlates
- Genomic Instability → DNA repair support: NAD+, B vitamins, antioxidant network
- Telomere Attrition → Telomerase activation: TA-65, astragalus, stress management
- Epigenetic Alterations → Methylation support: methylated B vitamins, DIM, resveratrol
- Loss of Proteostasis → Autophagy activation: fasting, rapamycin, spermidine
- Deregulated Nutrient Sensing → Insulin sensitivity: inositol, Mg2+, metformin, berberine
- Mitochondrial Dysfunction → CoQ10, NAD+, liposomal delivery optimization
- Cellular Senescence → Senolytics: fisetin, quercetin (dasatinib for clinical use)
- Stem Cell Exhaustion → GH secretagogues, peptide BPC-157, PRP support
- Altered Intercellular Communication → Anti-inflammatory: omega-3, glutathione, vagal tone optimization
- Disabled Macroautophagy → Autophagy induction: intermittent fasting, spermidine, EGCG
- Chronic Inflammation → Resolution biology: EPA/DHA-derived SPMs, Thymosin Alpha-1, anti-SASP senolytics
- Dysbiosis → Microbiome restoration: targeted probiotics, prebiotic fibers, gut barrier repair (glutamine, BPC-157)
The AXEL Protocol Connection
This 12-hallmark framework directly informs AXEL's TerraMatrix™ terrain system. Each of the seven terrain domains maps onto one or more hallmarks — allowing consultants to identify which molecular aging processes are most active in a given patient and prioritize interventions accordingly. Rather than a generic 'longevity stack,' AXEL protocols target the specific hallmarks showing highest biological activity in each individual.
